ABSTRACT

Objective To investigate the incidence of postoperation constipation in patients who experienced thoracolumbar fractures screw fixation surgery.Methods A prospective cohort study was performed by investigating 99 cases who experienced thoracolumbar fractures screw fixation surgery.The patients were followed up for 1 month.The postoperative constipation relief situation was observed.Kaplan-Meier method was used to draw a plot of survival,Cox proportional hazards regression model analysis was adopted for single factor and multiple factors analysis and the prediction model of constipation relief after surgery was established.Results The trend of postoperative constipation relief of patients showed a trend of first increased and then decreased,the number of cases reached high peak on the second week,the constipation relief median time was (14.00±0.76) days.Cox multiple factors regression analysis showed that prediction function model of postoperative constipation relief was h(t)=[h0(t)]e(-0.826X1+0.353X2+0.381X3-1.404X4).Conclusions There was a high incidence of constipation in patients with thora-columbar fractures after surgery and it is difficult to relieve,clinical nurses should pay more attention to the influencing factors of postoperative constipation relief and help patients reestablish normal bowel movement as soon as possible.

ABSTRACT

<p><b>OBJECTIVE</b>To analyze the epidemiological characteristics and the bacterial pathogen composition of pneumonia among children under 5 years old in Ningbo.</p><p><b>METHODS</b>In 2013 February-April, we selected pediatric ward in three hospitals according to stratified cluster random sampling, and from which we collected all 57 556 hospitalized cases in January 2009-December 2012 period of children under 5 years old. A total of 16 740 medical records and bacterial spectrum records of pneumonia or bronchial pneumonia cases among children under 5 years old were described to calculate the proportions of children pneumonia or bronchial pneumonia hospitalized cases accounted for hospitalized children over the same period(referred to as proportions of pneumonia incidence), the detection rate of bacterial pathogens, proportions of bacterial spectrum, and to analyze the characteristics and bacterial pathogen composition of pneumonia among children under 5 years.</p><p><b>RESULTS</b>The proportions of children pneumonia in 2009-2012 were 26.16% (2 882/11 017), 31.23% (4 004/12 819), 29.35% (4 898/16 689) and 29.10% (4 956/17 031)(Z = 3.13, P < 0.01), and were in a downward trend by the increasing of age(Z = -113.74, P < 0.01). The proportions of children pneumonia of 0-5 age group were 50.71% (6 756/13 318), 26.60% (3 159/11 877), 24.17% (2 574 /10 648), 22.36% (2 509 /11 223) and 16.63% (1 745 /10 490). The proportions of children pneumonia with different season were 26.90% (3 725/13 850), 26.51% (3 788/14 287), 29.59% (4 442/15 011) and 33.21% (4 785 /14 408) (χ² = 198.77, P < 0.01). The proportions of severe children pneumonia of 0-5 age group were 1.10% (74/6 753), 0.47% (15/3 159), 0.19% (5 /2 574),0.08% (2 /2 509) and 0.06% (1 /1 745), with a downward trend by the age(F = 57.62, P < 0.01). The detection rate of Klebsiella pneumoniae, Haemophilus influenzae,Acinetobacter baumannii, Escherichia coli, Staphylococcus aureus, and Streptococcus pneumoniae was 19.3% (540/2 805), 16.4% (460/2 805), 13.6% (381/2 805), 11.9% (335/2 805), 11.5% (323/2 805) and 6.8% (192/2 805).</p><p><b>CONCLUSION</b>In 2009-2012, the proportions of children pneumonia under 5 year old constituted a relatively high proportion in Ningbo city. Children under 1 year old were the major suffering group of pneumonia and severe pneumonia, which should be the key prevention group. The major bacterial pathogens of children's pneumonia in Ningbo are gram-negative bacteria.</p>

ABSTRACT

OBJECTIVE To investigate the drug resistance of 554 Enterococcus isolated in our hospital. METHODS The clinical isolates were identified and antimicrobial susceptibility tests were detected by VITEK-60 of Bio-Merieux of France. RESULTS Totally 554 Enterococcus strains were isolated,including 361 E. faecalis (65.2%),143 E. faecium (25.8%),22 E. gallinarum (4.0%),18 E. aviums (3.2%) and 10 other Enterococcus isolates (1.8%). For E. faecalis,the resistance rates to ampicillin and penicillin were lower than the average,(1.1% and 5.3%),which can be chosed as the first selective drugs in clinical therapy for E. faecium,the resistance rates to ampicillin and penicillin both were over 74%,they should be carefully used in therapy. but quinupristin/dalfopristin and linezolid were the best choice.Curing infections caused by the other Enterococcus except E. faecalis and E. faecium,linezolid should be the first selective drug in clinical therapy. CONCLUSIONS While curing infections caused by multi-resistant Enterococcus,doctors should choose proper therapy and according to difference of their drug resistance among Enterococcus spp,in order to prevent appearance of drug-resistant bacteria.

ABSTRACT

OBJECTIVE To study the incidence of ?-lactamases and oxacillin-resistant Staphylococcus aureus and their drug resistance in our hospital. METHODS The clinical isolates were identified by VITEK-60 of Bio-Merieux of France. K-B agar diffusion method and confirmed test were used to detect the drug resistance of oxacillin-resistant S. aureus. RESULTS Among 175 S. aureus isolates, the incidence of ?-lactamases was 94.3% and the incidence of oxacillin-resistant S. aureus was 28.6%. Oxacillin-resistant S. aureus was mostly isolated from pus/wound (52.0%), then from sputum and throat swab (32.0%). CONCLUSIONS To cure infections caused by ?-lactamases-noproducing S. aureus, penicillin is the best choice. For infections caused by ?-lactamases-producing but oxacillin sensitive S. aureus, the first generation cephalosporins are the best choice, while curing infections caused by ?-lactamases-producing and oxacillin- resistanct, S. aureus nitrofurantoin and linezolid are the best choice. The resistance rate to vancomycin is zero. It should be carefully used in therapy.

ABSTRACT

OBJECTIVE To study the clinical distribution and the drug resistance of extended-spectrum ?-lactamases (ESBLs) producing Klebsiella pneumoniae and Escherichia coli. METHODS The clinical isolates were identified by VITEK-60 of Bio-Merieux of France. K-B agar diffusion method and confirmed test were used to detect the drug resistance of ESBLs-producing K. pneumoniae and E. coli. RESULTS The incidence of ESBLs was 52.8% in E. coli and 63.5% in K. pneumoniae. E. coli was mostly isolated from urine (22.1%). The second was isolated from pus/wound (11.7%). Majority strains were isolated from surgical and pediatric wards. K. pneumoniae was mostly isolated from sputum and throat swab (37.8%), 21.8% from neonate ward and 16% from child ICU. CONCLUSIONS To cure infections caused by ESBLs-producing K. pneumoniae and E. coli, imipenem is the best choice and piperacillin/tazobactam is the second choice.

ABSTRACT

Objective To improve the quality of first aid,raise the working efficiency of nurses,make the management of endemic area crash truck more standard,scientific and simple and provide swift medical care to patients in emergency.Methods An endemic area crash truck is developed in accordance with the rule and regime of first aid item.Results Endemic area crash truck was given a national practical newtype patent and was carried out in hospitals.Conclusion Endemic area crash truck has a reasonable construction and can be locked by one time lock.It greatly shorten the time for handing over and counting by nurses,which elevates the efficiency of nurses and guarantee the management quality of first aid item.
